We know that personalised support can be transformative, and that Future Pathway makes a positive difference to people’s lives.
We are also hearing that too often, it’s a struggle. Resources may exist, but they are not always accessible to those who need them.
This needs to change. This report shares what we are learning about what works and doesn’t work for people, striving for improvement so that more people experience support that really works for them.
